Men who experience problems getting and maintaining erections should consider working with a health professional to treat the underlying cause. Many health conditions can reduce blood flow to the penis, including heart disease and diabetes.
Oral medications such as Viagra or Cialis are typically the first treatments doctors recommend because they are proven and relatively safe. However, they are not effective in all cases.
Alprostadil
Alprostadil is a synthetic form of prostaglandin E1, a naturally occurring substance that works to dilate blood vessels. This allows for more blood flow to the penis, resulting in an erection. It is injected directly into the corpus cavernosum and works within minutes. It is usually only used when other treatments have failed and is often paired with psychological therapy.
Phosphodiesterase type 5 inhibitors are the first-line treatment for erectile dysfunction (ED). However,Tadalista 60 cannot be used by a significant subset of men due to side effects and drug-drug interactions. This is the rationale for developing second-line therapies such as intracavernosal alprostadil. Its mechanism of action is different from that of PDE5 inhibitors, and clinical trials have shown that it is effective in a subset of ED patients who fail to respond to oral PDE5 inhibitors alone.
A single-blind dose-escalation study of 683 men was undertaken at 33 sites to establish the optimal dosage of intracavernous alprostadil for home self-injection and to evaluate safety and feasibility. The causes of ED were assessed by history and physical examination, duplex or color Doppler ultrasound of the corpora cavernosa, and penile biothesiometry. Adequate doses of alprostadil induced an erection sufficient for intercourse and lasted 60 minutes or more for 389 (57 percent) of the men.
This was superior to that produced by papaverine, the combination of papaverine and phentolamine, and topical nitroglycerin in terms of both clinical and RigiScan evaluations. The most commonly selected dose of alprostadil for home injection was 250 micrograms, which was associated with a 2.5-point improvement in the 30-point International Index of Erectile Function domain score and an average increase in penile diameter of 15 percent.
Vacuum Pump
Penis pumps, also called vacuum erection devices or VCDs, fit over the base of the penis and draw blood into the area to create an erection. They have a plastic tube, a battery or hand-powered pump and a constriction ring that fits over the base of the penis after it gets erect. The ring keeps the erection firm and shouldn't be left on for more than 30 minutes because it could cut off blood flow to the penis and cause injury (FDA, 2018).
Penis pump therapy isnt a cure for ED and might only help you have a good erection during sexual activity. You still need to take oral prescription ED medicines, such as sildenafil or tadalafil, and treat the underlying condition that's causing your ED.
You should only use a penis pump under the supervision of a urologist, as it may not be safe for some people, including men with diabetes or spinal cord injuries. Penile implants have a high satisfaction rate, are generally covered by insurance, and provide a long-lasting, firm erection. However, they require a longer recovery period and can be expensive. A doctor will typically only prescribe a penile implant once less invasive treatments, such as oral medication, have been tried.
Before undergoing surgery, a man will have a physical exam and a urologic exam. This will involve examining the prostate and genitals, as well as getting a urine sample. A medical history will also be reviewed. The man may be required to stop taking some medications and supplements, including blood thinners, anti-inflammatories, and herbal remedies.
The operation is performed in the operating room under either general or spinal anesthesia. It usually takes between one and two hours. The man will be instructed to apply an ice pack to the area afterward to decrease pain and swelling. Oral antibiotics will likely be given to reduce the risk of infection.
The most common type of implant is an inflatable device that consists of a pair of cylinders in the erectile tissue and a pump in the scrotum with a reservoir of saline. Squeezing the pump in the scrotum moves the saline into the cylinders to create an erection. Deflating the device allows the cylinders to return to a flaccid state.
Lifestyle Changes
Many men can find relief from erectile dysfunction by making healthy changes to their diet, exercise, and lifestyle. They may also benefit from sex therapy.
A healthcare provider will evaluate a man with ED by asking questions about his symptoms, doing a physical exam, and ordering lab tests. They will also check for heart and blood vessel problems, as well as look for other causes of ED like side effects from drugs or psychological factors.
